The Smart Vision Quiz is a parent observation checklist for discussing concerns with our team at Bondi Beach or Mosman. It is not an eye test, a diagnosis or a way to rule out an eye condition. You can book an appointment without completing it.

What is the Smart Vision Quiz?

Use the checklist to record changes you have noticed in your child’s comfort, vision or everyday activities. The observations can help start a conversation; they do not establish the cause of a difficulty or determine whether treatment is needed.

How to use the checklist

Select the observations that apply, enter your contact details and indicate whether you would like a callback. Read the form’s communication notice and our privacy policy before submitting. Do not include additional medical records or sensitive details in this form.

Understanding its limits

Reading, spelling, attention and coordination difficulties are not specific to eye conditions. An eye examination can identify coexisting eye problems, but it does not replace educational or developmental assessment. Discuss learning concerns with your child’s teacher and appropriate health or education professionals; do not postpone evidence-based educational support while seeking eye care.

    Questions about the checklist

    Can this quiz diagnose an eye condition?

    No. A checklist cannot measure eyesight, examine the eyes or diagnose an eye condition. An optometrist needs to assess your child directly.

    Do reading or attention difficulties prove there is a vision problem?

    No. These difficulties have several possible causes. An eye examination can identify coexisting eye conditions, but dyslexia and learning difficulties require appropriate educational assessment and support.

    Should I wait for a quiz response before booking?

    No. If you are concerned, contact the practice directly. Do not use a quiz response to delay an eye examination or educational support.
